    1.     Brief survey of organization

    1.1.         Organization Profile

    Public Organization «YGT» was created as result of voluntary consolidation of youth people, leaders and activists, which learned skills work with youth people and socials work in leading international and local public organizations such as UNICEF, PSI, JA, YP, and PO of STAI. Energy and enthusiasm of workers of organization aimed to achieve better world for children and youth.

               Public Organization «YGT» was registered on January 18, 2008 under number 004-03 IJ in Tajikistan Sughd Region Justice Department. Then organization was reregistered on March 25, 2011 in Tajikistan Ministry of Justice under number №1373 IJ and received republican status.

    1.2.         Vision, mission, values and organization goals

    Our vision, world, in which young people become one of the main participants for society development putting their potential for its improvement.

    Our missionconsists in improving youth people’s potential for their further participations in the processes of developing society and decreasing poverty rate.

     We believe that it’s important to consider youth opinion, and encourage their participation in society and country.Youth is strategic resources for every country, especially for developing countries, because of their enthusiasm, energy and fresh ideas. In view of that youth up to 30, is equal to 70 % Republic population and stirring up its role in society life is topical and does not endure declining tasks. Youth is precious human resources that are necessary to promote further development as in the economic, social, and cultural sphere and in the matter of society solidarity as well.

    We valueyouthinvaluable social and creative power, its ability of forming modern approaches, finding the ways of solving its problems, impacting on prevention the daily problems of growing up generation in general and its positive, ambitious ideas, competent constructive participation and its self-realization in all sphere of social life.

    1.     Strategic plan meaning

     

    The strategic plan of“UGT” Public organization defines the essential directions of organization development for 2012-2015 in the matter of children, young people saving and development health. It is consonant with priority subject directions of the government youth policy, Tajikistan Republic national development strategy till 2015, poverty lowering strategy for 2010-2012 and other national strategies/programs. The strategic plan is based on the analyses of the following factors and tendency that concern Tajikistan children and youth health, their problems and needs in this sphere and also on the experience of other initiatives in this direction:

     

    2.1.                       Demographic situation

    According population Tajikistan is one of young countries in the world, as 70% of its population are young people up to 30. Middle age of Tajikistan population is 24,7 (Annual statistic RT for 2008 and government statistic of youth policy).

    In relation to sex 49,8% of youth is girls, which have direct relation to the demographic opportunities.

    93% of Tajikistan territory is mountains and more than half population living at the countryside. Accordingly, 70,4% of youth is village youth. That’s why the problems of health spread that youth category, because they have limited opportunities to get necessary service.

     

    2.2.                       Political factor

    At all times of developing societies to the youth policy as strategic and priority sector of government is paying special attention.

    Youth is the subject of national-state interests and the main factors of supporting the state and society development. The future of new generation should be inseparably connected with the goals of keeping, developing and revival of the country, succession of historical and culture heritage.

    From positions of youth in socio-political life, and its confidence in the future and activity will depend on the pace of Tajikistan development in democratic reorganizations.

    Precisely young people should be ready to resist global challenges.

    With the goals of the teenagers and youth health protection,the Government resolution of RT from October 30, 2010 under number №561 adopted the programme of youth health development in RT for 2011-2013. The realization of this programme will contribute to achieve the MDG and solve the problem of the youth health improving. Реализация данной Программы способствует достижению Целей развития тысячелетия, решению проблем улучшения здоровья молодежи.

    The main goal of programme is improving the state of young people health by decreasing the influence of risky behavior and bad habits, as casual sexual affairs, use of drugs, STI and HIV/AIDS.

     

    2.3.   Economic factor

    Despite the growth of budgetary financing and external aids the limit of access to service in public health sector caused by a whole number of reasons which divided in three groups: specific – causing the high infant and maternal mortality, and also spreading socially dangerous infection; systemic – concerning not high speed of reform, the limit participation of private sector, lack of priority by using tools, financing management; technical – exceed the limits of public health sector and needing the multi-sectorial state approach.

    For providing stable activities and health development granted budgetary funds is not enough.Public health system does not have all the necessary things for successful implementation its key functions. Government expenses for public health sector decreased from 4,5% of GDP in 1991 to 1,9% in 2009.

    Public health system becoming more and more depend on private outgoings for providing medical service and external aids. There are lowering of qualification level and lack of medical workers especially in countryside caused by ineffective planning, placement and management of human resources, also the weak motivation, low wages and connected with it the high outflowing of medical workers.   The problems becoming worsen, because of not providing modern equipments and technics to medical institutions and neglecting the facilities and building conditions.

    2.4.   Social factor

    Health is precondition in sphere of improving the social-economic situation and development of personality. Rights for health to available, effective and friendly public health service are one of fundamental human rights. One of the main strategic government tasks, the whole world and Tajikistan also is formation the healthy young generation.

    Tajikistan public health system is the key sector for achieving MDG, as efforts for improving public health (highlighting special attention to the children and youth) will further for sustained economic growth and human potential development of the country.

    Despite of achieving results in sphere of keeping the children and youth health because of absence systemic approach of programme actions and properly government financing below mentioned problems decreasing the tempo health development above mentioned groups and the whole population.

     

    2.4.1. Situation with HIV/AIDS

    According monitoring data HIV growth in the end of 2011 in RT 3846 incidents was registered with HIV-infections (data from Public Health Ministry of RT).

    Colorations of HIV infected the youth up to 30 is 45,7%. In relations to youth up to 35 is 60,3%, which increased in 9,1% against 2008. 1,9% infected is the children up to 14. It means that the age of HIV infected annual rejuvenated, which can bring to the threat of national gene pond.

    Data about official registered HIV accidents indicate that the main way of infection is injection. Many (55,1%) infected by this way, 27,2% infected by sexual relationship.

    According data of national report about the course of implementation declaration on commitment to struggling with HIV/AIDS in special session UN General Assembly, in 2011 just 13,34% of the youth (15-24 years) knows about HIV prevention (Research on households among population age groups 15-49, 2010, Research Strategic Centre under president of RT). 

    Above mentioned data shows the reality of incidents and requires more attention from responsible persons for HIV prevention and other interconnected problems.

    2.4.2. Situation of drug use

    RT about geographical positions is transboundary country for drugs transportation, which facilitate the growth of drug use.

    According to official statistics (Public Health Ministry of RT) in July 1, 2010 was registered 7347 incidents of drug use, 37,7%  is the youth at the ages of 18-29.

    According to official data spreading of HIV among the drug use which was 16% in 2005 and increased up to 17,3% in 2009. This little growth indicates that HIV situation in RT has epidemiological character. Every year becomes harder to prevent the spreading of this disease.

    A special risky group of drug users is the group of persons who render sex works and injection drug users. A part of this group owned means spend to purchase the drug use. This condition promotes the growth of HIV infection.   

    Also, many incidents of drug addiction, drug business occurs among the youth who lives in border region in Islamic Republic of Afghanistan, which worries our society.

    2.4.3. Bad habits including STI among the youth

    Human becoming the cause of emerging many diseases: the low level of culture, unhealthy life style and bad relations. According to data of WHO human health (55%) related to the healthy life style. Unfortunately not everybody could really realize this description.

    There are such situations in condition of RT, which leads to bad habits. Use of cigarette and tobacco for citizens of RT including the youth is the common behaves, this phenomena becomes common habits, which is worriedly.

    The government of RT pays attention to ban use of cigarette in public places and this is timely measures and its adoption needed regular propaganda.

    Another bad habit which needs special attention is vagrancy and indecent behavior of teenagers. There is not any special statistical system comparatively to this tendency in the country and their registration carrying out by different appropriate bodies, which does not have completely information about the residence of such teenagers. According data of research about 13 thousand teenagers in the country doing vagrancy in such big cities as; Dushanbe, Khujand, Kurgan-tube, Kulyab and others. Indecent behavior of such society category generally is begging, car washing, sniffing glue and gasoline and etc, in big streets, public places, in big markets and shops.

    Prostitution and commercial sexual relation also is indecent behavior, but for certain part of young girls it becomes regular habits. Comparatively to this phenomenon there is not any special information service for identification of personality and its purpose to bad habits.

    According to official data of Internal Affairs Ministry of RT in the country more than 1100 (in Dushanbe city 225) young girls doing prostitution, many of them unemployed or it is their constant activity. According to spreading of HIV data 8000 individuals in 2003 and 12500 in 2009  doing commercial sex. First of all this growth is the reason that young generation not paying enough attention for development their life activity and the low level of social opportunities every individual.

    According to data of the epidemiological inspectorate 2,8% of sex workers in 2008 was infected by HIV. Besides, drug user women by injection most probably can be infected by HIV. According to official data 11% of drug user by injection is women.

    The organization level of carrying out actions on prevention of prostitution does not answer the requirements of reality. The main reason of spreading HIV based on not awareness of our society about this disease incident.

    During 2005-2006 HIV incidents among commercial sex workers increased from 0,7% up t0 3,7%.

    In 2008 25,5% HIV incidents was infected by sexual relation.

    Furthermore, many such incidents as STI among commercial sex workers including syphilis incidents are 12%.

    2.4.4. Reproductive health and family planning

    In the last decades in the many countries of the world is observing the acceleration process. The consequence of mental physical the acceleration ordinary is the early beginning of sexual relation. At the same time, the low level of thinking in sexual education sphere and life skills on health issues negatively effects on reproductive health of teenagers, and helping the nascency of unwanted pregnancy, increasing the risk of infection by STI and HIV/AIDS.

    According to data of research (UN, 2005) is observing the early marriage. About 1% of the young women in the age 15-49 confirmed their marriage in the age up to 15.  But, 15% of the young women at the age 20-49 married up to 18 ages. The early marriage usually happens among undereducated women, which have just primary education.

    According to the information of Reproductive Health Centre in Dushanbe city on teenagers pregnancy distribution found out, that 0,7% of childbirth happen at the 14-15 ages, 2,5% at the 16-17 ages, and 8,2% at the 18-19 ages. There is the growth of teenager’s pregnancy and its decreasing is not taking shape yet. Also, the cause of this phenomenon is that the access to the medical service ant the level of teenager’s awareness about family planning really low, and this is provoking anxiety.

    Undoubtedly, teenager’s pregnancy is unwanted and 80-90% incidents end by abortion. From the general number of abortion 11% is the youth at the 20 age. Also, the growth of abortion up to 2,1% is observing among individuals at the 15-20 ages.

    Generally, the reasons of complications on teenager’s reproductive health are the low level of their education and concerning about personal life.

    According to UN research 22,3% teenager’s confirmed that the consequences of the abortion can be infertility, but 73,6% have difficulties to answer (haven’t enough information) and 4,1% commented that the abortion consequences cannot be harmful.

    The results of the UN research among 7-8 forms in 2006 shows that for 55,3% teenager’s the main information source is the TV and radio, 23,5% from schools and just 2,6% from family.

    But according to data on research of Youth, Sport and Tourism Committee under the Government of RT conducted in 2008 shows that the main information source for about 53% teenager’s is their peers and 5,7% from parents and teachers.

    Strategic plan defines the goals and tasks of organizations for planning period, methods of achieving planned tasks and implementation of planning tasks and also services, which will be provided to beneficiaries expected results and stability of plan.

     

                                                                                              

    3.              Activity analysis of the PO «YGT» for 2008-2011

    Analyzing advantages and disadvantages of the organization concluded the followings;

    Advantages:

    v The organization is leading by competent direction which has ideas, enthusiasm, aspiration to high aims and the great experience in other

    v Managers and volunteers team working with different groups of beneficiaries, mainly from among children and youth;

    v Generally actions aimed to solve different problems mainly connected with the health;

    v Organization is authorized member of:

    -       International network of women and youth organizations working on matter of sustainable development and health protection from bad influence of environment;

    -       National coordinating council on health protection of the youth;

    -       National networks of organization-partners on water supply and sanitation;

    -       National council on development of volunteerism;

    -       Public council under chairman of Sughd Region and Founder of Republican association on diseases protection of social characters, which make it possible to increase the quantity and quality of service for PA;

    v The organization has main funds such as office equipment and furniture’s;

    v Gained authority among state, international and public organizations, which let lobby and promote interests of the children and youth in different levels.  

     

    Disadvantages:

     

    v Absence of the own office facilities;

    v Short-term projects;

    v Lack of internal programs on development of the cadre potential;

    v The low technical office facilities.

     

    Summarizing all above mentioned can conclude that activity of the organization shouldn’t change its main course and directed to solve social problems connected with the health. But taking into consideration new spheres where necessary to make a contribution on development and solving society problems, especially children and youth, then should add to main action plan followings:

     

    Ø Advocacy direction;

    Ø Help on development of orphan child;

    Ø Working-out the programme development of the cadre potential;

    Ø Gaining fundraising in direction of getting own office building and technical organization facilities.

     

    4.  Why is special attention to the children and youth health?

    Before to answer to this question we should know the followings:

    Definition of the «health» conception:

    Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity ¹.

    (¹ According to the definition of WHO)

    Health as the main human right

    According to above mentioned definition, human health is universal aim.

    Everyone wins when there is fairness attitude to the health.

    In WHO Charter (21) said: “Possession the highest achievable health level is one of the main every human right without differences of race, religion, political beliefs, economical and social status”.

     

    Summarizing we can say that giving opportunity to the children and youth to preserve their health, we supporting their legal right in this sphere. Having the complete physical, mental and social well-being, children and youth can become personality who is not indifferent to destiny of the other people and the nation in whole. 

     

    5.   Strategical goals, tasks, actions and expected results concerning mission  achievements of the organization for 2012-2015 are followings:

    During next 4 years the organization planning to achieve the following goals:

    Goal № 1:

    Improve the quality and expend the quantity of organization service planning to provide during this plan to 2500 children and youth with regard to gender (50/50).

     

    Task 1.1.

    Strengthen 26 administrative and 12 programme skills (X) workers of the organization for provision 13 kinds of programme services mentioned below in the task 3.1 and 4.1., also achieving the goals 2-6.

    During the year carried out 3 assessments of 10 workers activities in the network of common and individual HR-plans, assignments of 14 programme service and done measures on achievement of goals 5 and 6.

    GOAL № 2:

    Strengthen organization cadre during plan for giving 13 programme services to children and youth up to 30.

    Form positive civil positions of 20 youth 14-30 ages with regard to the gender 50/50 in relation with voluntary activity by its comprehensive participation and self-realization in organization actions.

    - The main goal of talk-show: attract attention and raise confidence of other person who makes decision in importance of involving youth in making decisions process concerning their sound well-being.

    Develop, make copies and distribute 500 copies of program report on the results of task 4, "Example of the youth advocacy actions to improve universal access to HIV prevention and other social diseases for young people, through the integration the principle of youth participation in the planning, working out, development, implementation, monitoring and evaluation of programs to prevent drug abuse and HIV infection among young people".

    There was held a three-day workshop and 20 employees and strategic partners were trained the organization of advocacy skills and integration of youth participation in all phases of the programs / projects to prevent the spread of HIV infection and the maintenance of health.

    Studied and analyzed 15 normative legal acts and developed a brief overview of the legal framework supporting the need for the integration of the principle of youth participation.

Electronic versions of the best youth practices in drug prevention, HIV, and other topics of healthy lifestyles accumulated via e-mail, ​​entered to the electronic collection and 150 copies in the Tajik, Russian and English language distributed to forum participants.

    1.     Stability plan

    The current strategic plan is the first in the history of the NGO "YGT". In this regard for its implementation and identification of new targets after its completion the organization should consider the following:

    - strengthening the staff of (X) employees and their personal and professional qualities;

    - obtain by request from government authorities, in particular the Committee on Youth, Sports and Tourism the office building free of long-term use (at least 4 years) in order to reduce the costs of office rent on account of the rise for the above program needs;

    - get additional office equipment and furniture for at least 75% making up of the their staff and office building of the organization;

    - expand the sources of funding by foreign funds, profitable companies, sponsors, donations and income generating activities;

    - seek out opportunities of maximum coverage program and administrative costs through donations of service providers (eg: Internet) and effective use of the various benefits provided by the state (eg: VAT exemption).
